It looks like the app embed is not enabled. Because of this, the RFQ page contents are not visible. Please contact the store owner to enable it from Theme App Embeds.
Please go to Settings tab of CWS Request Quote ‑ Hide Price App then set "Yes" for "Enable Markets Feature for RFQ App". If you're not sure then please drop an email to App support team.
